*Must be age 4 by July 31 , to attend CELC* 

Step 1: Click on this link to access and fill out our Pre-K & Kindergarten online enrollment form. This initial form will ask you to upload your proof of residency (utility bill, tax receipt, lease agreement, affidavit of residency, etc), your child's immunization records, and your child's birth certificate. All must be provided to CELC in some form before enrollment.

Step 2: Contact the Chillicothe Early Learning Center (CELC) by calling 660-240-3316 or by email at lholder@chillicotheschools.org. (Please use email during summer hours.) 

Step 3: Complete ALL forms required by CELC. Forms can be picked up at CELC during school hours, or printed online. Click on this link to fill out the parent forms. You can print the forms after filling them out, or save them and send them to lholder@chillicotheschools.org. Click here to access the Medical Form to print out. This form must be completed by your child's healthcare provider and turned back into CELC. You may also scan and email this form to lholder@chillicotheschools.org.

Step 4: Confirm with CELC that the above steps are complete and then complete the online Registration Update in August, once it is available to all Chillicothe R-II School District parents.
