Chillicothe R-II Employee Injury & Workers Compensation Information

Guides are provided below for both the Employee and the Administrator/School Nurse.  Please follow the instructions provided for all workplace illnesses or injuries.  

If an employee has an injury that is life, limb, or eyesight threatening, they are authorized to seek Emergency Treatment at the nearest ER.  All other injuries must be assessed by the Triage Nurse and their recommendations followed.  

Even if the injury warrants an ER Room visit, the Building Administrator and/or nurse are still responsible for providing all required forms for this visit. Copies of these forms should immediately be provided to HR.

Care Instructions, including medical treatment recommendations through the Triage Nurse, DOES NOT guarantee acceptance of the claim as a workers compensation injury.  Gallagher will make that determination based on whether the injury is work related.  

Employee will be responsible for medical care expenses in the event the claim is deemed not work related.

Any and all follow up care and/or treatment must be authorized by Gallagher Bassett.  Physicians may find the authorization contact information at the bottom of the MUSIC Authorization to Treat form. Any unauthorized treatment is subject to denial and the expense may be the responsibility of the employee.
